Chloe Ubah (Registered Manager & Nominated Individual)

I have nearly 15 years experience in Health and Social Care. I have worked in a variety of services such as a Dementia care home to Adult Social Services.

Whilst working in the Dementia care home I was able to complete my Level 3 Diploma in Health and Social Care and became a Senior Care Assistant. I supported new staff with their inductions and would complete medication competency’s with staff. I was also the Activities Co-ordinator for the home and would create exciting activities for the residents to be part of. Due to the variation in the service user’s abilities, activities were adjusted to ensure all service users were included and were able to complete the activity too.

Later on I was given an opportunity to work for a Local Authority in the Adult Social Care Sector along side Social Workers, AMHP’s and other professionals. This supported my growth in knowledge and learning. I worked in the ‘front door’ of adult social services where referalls for Care Act Assessments were triaged and acted upon accordingly.

I then moved to the Shared Lives Service as a Shared Lives Officer. This is a service which provides family based care to adults with Learning Disabilities, Mental Health, Physical Disabilities & Dementia. The service user moves into a registered carer’s home and support is provided. I manage the Shared Lives Carer’s in my caseload and support the service users by ensuring they receive the best possible care.

As the Registered Manager I am in the process of starting my Level 5 Diploma in leadership for health and social care and I am undertaking refresher courses in safeguarding adults, complaints handling, medication, consent, mental capacity, health and safety and human resources.
